Good looking foolproof contact form?

I know there are countess email forms available out there on the internet... but I have yet to find one that is modifiable and easily implementable into my own site. Smashing magazine featured an article in 2007 (http://www.smashingmagazine.com/2006/11 ... solutions/) but they all seem a but clunky, convoluted and not terribly beautiful. Can someone point me towards a modern looking and pretty contact form with a foolproof backend....

What do you mean, foolproof backend? You want track and log IPs?
